My physician performed a sacral nerve destruction at S1, S2, S3 and S4. I am not certain how to code this. Should I report 64640 only once?

You should report 64640 (Destruction by neurolytic agent, other peripheral nerve or branch) for each level. You should report 64640 x 4 (4 units).
