I am new to pain management coding and have a question. What is the correct way to bill for a transforaminal injection of L3-L4 and L4-5? He is injecting L3-L4 bilaterally and L3-L4 on one side. Can I report 64483 three times?

No, you would report 64483 (first level) and since it is performed bilaterally you would append Modifier 50 to CPT 64483. For the second level (L4-L5) you would report the add-on code 64484 for the second level. This procedure includes imaging guidance fluoroscopy or CT guidance and is not reported in addition to the procedure.
